Blepharoplasty is a surgical procedure to remove some of the excess skin or fat from the upper or lower eyelids to reshape them. An upper blepharoplasty can improve hooded, droopy or sagging eyelids and lower eyelid surgery can reduce wrinkles, puffiness and eye bags.

With age, our muscles slacken and the skin loses its elasticity. For the eyelids, this results in folds in the upper lids and deepening creases in the lower lids. Also, the slackening of muscle beneath the skin allows the fat which cushions the eyes in their sockets to bulge forwards to give the appearance of eye bags.

Why undergo eyelid surgery?

Blepharoplasty is most commonly performed to improve the 'tired' look associated with heavy, drooping eyelids. Eye lift surgery can help to refresh the appearance of your eyes and improve the constantly exhausted and tired appearance of undereye bags.

For some patients, blepharoplasty can also improve the symmetry of the eyes. Another issue some people face is sagging upper eyelid skin that obstructs the visual field, impacting your lateral vision. Blepharoplasty eyelid surgery can remove this obstruction and help to improve vision.

How is blepharoplasty surgery performed?

Blepharoplasty surgery is typically performed under local anaesthetic, but can also be performed under twilight anaesthetic. For upper blepharoplasty, incisions are made in the natural folds of the upper eyelid; in the creases of the upper lids, and extend into the corner of the eyes. These are positioned so that they are well concealed within the natural structures of the eyelid region. Next, fat deposits can be repositioned, the muscles can be tightened and excess skin can be removed.

For lower blepharoplasty, the incision is made just below the lower lash line, allowing excess skin to be removed, and excess fat to be repositioned or removed.

How long does recovery take after blepharoplasty surgery

You should typically expect to need around 1-2 weeks off work after surgery to recover from the operation. You will also not be able to drive for a few days following surgery.

Instructions on discharge
  • Hypromellose artificial tear eye drops to be used throughout the day every 4 hours or whenever necessary for 2 weeks.
  • Chloramphenicol eye ointment to be put into the eye at night for 2 weeks.
  • Washing of the face is allowed but be gentle around the eyes where the tapes are as they help to hold the stitches in place. The tapes need to stay in place until the first dressing check.
  • Ice compress to the eyes for 2 days.
  • TED stockings to be worn for 2 weeks.
  • Sleep with head slightly elevated and avoid bending the head down.
  • Discharge with standard painkillers.
Dressings Clinic
  • Usually about a week after the surgery when the sutures around the eyes are removed and another tape applied to the outer scars which can be removed at the end of the second week.
Activities
  • No restrictions but avoid strenuous activities for about the first 2 weeks.
Wound Care for First 3 Months
  • Moisturise all scars for at least 3 months.
  • Makeup can be applied after the second week.
  • Avoid direct sunlight for the first 3 months.
