Eligibility for gender affirming surgery is more straightforward than many people expect. The most important step is booking a consultation, Mr Seni will assess your individual circumstances and answer any questions you have. As a general guide, suitable candidates typically:

Gender affirming breast augmentation is available to adults aged 18 and over.
✓ Have a diagnosis of gender incongruence
A formal diagnosis, typically from a GP, gender specialist, psychiatrist, or clinical psychologist, is required. A diagnosis made within the last 12 months is preferred by most private providers. If you don't yet have a diagnosis, we can advise on next steps at your consultation.
✓ Have a referral or supporting letter
A letter from your GP or a qualified gender specialist is required before proceeding to surgery. This confirms your diagnosis and confirms you have been assessed as suitable for surgical intervention. We can advise you on what this letter should include.
✓ Are in good general health
You should be in good physical and mental health. Mr Seni will carry out a full pre-operative assessment to confirm you are fit for surgery under general anaesthetic.
✓ Are a non-smoker (or are prepared to stop)
Smoking significantly affects healing and surgical outcomes. We ask patients to stop smoking for a minimum of six weeks before and after surgery.
✓ Have considered the role of hormone therapy
Hormone replacement therapy (HRT) is not an absolute requirement, but taking oestrogen for at least 12 months prior to surgery allows breast tissue to develop as much as possible before augmentation, which often leads to better results. If you have not started HRT, or have only recently begun, Mr Seni will discuss what this means for your specific options.

This depends on your anatomy, your goals, and how much donor fat is available. Implants offer reliable, predictable volume and are the most common choice for MTF breast augmentation. Fat transfer gives an entirely natural feel but is limited in the volume it can achieve (typically one cup size), and requires suitable donor areas. A composite approach — combining implants with fat transfer — gives the benefits of both. Mr Seni will give you a clear recommendation based on your specific anatomy at your consultation.
The procedure is performed under general anaesthetic and typically takes 1.5 to 2 hours. You'll be at the clinic for a few hours in total, including preparation and recovery from the anaesthetic, and will go home the same day.
Most patients are back to desk-based work within 1–2 weeks and feel largely normal within 4–6 weeks. Full recovery — including resolution of all swelling and implants settling into their final position — takes 3–6 months. Upper body exercise should be avoided for at least 6 weeks. Your specific recovery plan will be discussed at your pre-operative appointment.
Changes in nipple and breast sensation are a known risk of breast augmentation surgery. Many patients experience temporary numbness or heightened sensitivity after surgery, which usually improves as healing progresses. Permanent changes to sensation do occur in a minority of patients. In the absence of any complications, most patients can expect their implants to remain in good condition for 10–15 years — and there is no need to replace them at a set interval if there are no problems. Routine monitoring (such as periodic MRI or ultrasound) is recommended from around 10 years post-procedure. Mr Seni will advise you on long-term monitoring as part of your ongoing care.
Yes, in some circumstances. Not all patients are on HRT, and the eligibility for surgery is assessed individually. If you haven't started hormone therapy — or have chosen not to — Mr Seni will discuss how this affects your options and what results are realistically achievable. In some cases, starting HRT before surgery may be recommended to improve outcomes.
This depends on your chest dimensions, the amount of skin laxity you have, your existing breast tissue, and the implant or technique selected. During your consultation, Mr Seni will show you the range of sizes and profiles that suit your frame, and use his expertise to help you visualise likely outcomes. Realistic goal-setting is a key part of the consultation process — results should feel right for your body and proportions.
All surgery carries risk. The most common temporary effects are bruising, swelling, and soreness during recovery. Less common risks include infection, changes in sensation, asymmetry, and scarring. Rare but serious complications include capsular contracture and implant rupture. Mr Seni will explain all risks in full at your consultation and give you time to consider your decision.
