A bullhorn lip lift is a surgical procedure that shortens the philtrum, the area of skin between the base of the nose and the top of the upper lip. A fine strip of skin is removed through an incision hidden in the natural crease at the base of the nose, shaped like a bullhorn so it follows the contour beneath the nostrils. The lip is then gently lifted and the incision carefully closed.
By raising the lip a few millimetres, the procedure increases the amount of pink lip on show, can enhance the definition of the Cupid's bow, and creates a subtle, more youthful increase in the visibility of the upper teeth at rest. Unlike fillers or implants, it does this by improving the proportions of the lip rather than adding volume, so the result looks balanced and natural.
A bullhorn lip lift is often suited to people whose upper lip has lengthened with age, or who have a naturally long upper lip and would like more lip show. Because it reshapes proportion and gives a permanent result, it can be an alternative or a complement to lip fillers or a Permalip implant, something your surgeon can talk through with you. The procedure is performed under local anaesthetic, with the option of twilight sedation, as a day case taking around 45 to 60 minutes. The scar sits in the shadow at the base of the nose and typically settles to become discreet, and most people return to normal activities within a few days.

A lip lift changes the proportions of the lip by shortening the philtrum and revealing more of the upper lip, giving a permanent result. Lip fillers add temporary volume and are topped up over time. The two can be used together, and your surgeon will advise on the best approach at your consultation.
The incision is placed in the natural crease at the base of the nose, where it is well concealed. It is usually pink at first and fades over several months to become discreet. As with any surgery, healing varies from person to person, and scarring is one of the risks discussed at consultation.
The result is permanent, as a small amount of skin is removed and the lip is repositioned. The lips will still age naturally over time.
Most people take around 3 to 5 days off work. Swelling and tightness settle over 2 to 3 weeks. Sutures are removed at about 1 week, and we review you again at 3 months.
